If something is going wrong at COJUDE — fraud, harassment, a safety issue, theft, or a policy breach — this form is how you tell us. No login required. You can stay anonymous, or leave contact details if you'd like HR to follow up.

How this channel works

  • You can file with no login. The body of your report is encrypted at rest — only the HR review team can read it.
  • When you submit, you'll receive a reference token. Save it — it is the only way to look up your report's status later, and we cannot recover it for you.
  • Anonymous is the default. If you choose to share an email or phone number, it is encrypted at rest and visible only to HR on the case page.
  • You can check the case status at any time on the status lookup page with your reference token.

The report body is encrypted at rest. Only the HR review team can read it.

Contact (optional)

Leaving contact info is optional. If you do, HR can reach out to ask clarifying questions. Your contact info is encrypted at rest — only HR sees it on the case page.
